Key Responsibilities
Provide monthly care coordination through planning, facilitation, and advocacy for patient health needs
Maintain patient chart compliance through proper documentation and updates of medical history and care plans
Provide appropriate health education and escalate patient concerns to the triage nurse team
Qualifications
License and current registration to practice as a Licensed Practical Nurse in a COMPACT state
LPN degree from an approved program required
Pass background check
