Job Description - Minnesota Licensed RN Case Manager
A company is looking for an RN Specialist Complex Case Manager.
Key Responsibilities:
Engage with members, families, and providers to manage complex medical cases and develop comprehensive care plans
Conduct clinical assessments and collaborate with interdisciplinary teams to ensure effective care delivery
Monitor and update care plans while identifying cost-saving opportunities and community resources
Required Qualifications:
Registered nurse with a current MN license and no restrictions
5 years of relevant clinical care experience
CCM Certificate or ability to obtain within 3 years of starting the position
Experience in managed care is preferred
Flexibility to work varied hours
